Rural Health Network SCNY is assisting Delaware County Public Health, O’Connor Hospital, Delaware Valley Hospital, and Margaretville Hospital with the data collection for this year’s Community Health Assessment. This data will assist with the development of the county’s Community Health Improvement Plan and the hospitals’ Community Service Plans, all of which are intended to improve the health outcomes of Delaware County residents.
